Wet dream clarification contradiction

The text on the card is

If this is the first time you’ve played this, shout “My Familiar is mortally pregnant with twins!” Otherwise, destroy this and gain 2 random Familiars from outside the game

This text reads as you can either shout the phrase and keep  the Wet Dream in your deck, or not shout the phrase and destroy it for two random familiars.

However the card clarifications in the rule book states that if you do not shout the phrase, nothing happens. This clarification is the opposite of the how the cards reads.

I believe it goes like this: first play – say the first part, it does nothing else, this is your only chance to say it, second play- if part one was completed, do second part
